We are now electronic and thank you for your support as we move from paper to paperless.  Our new system, Centricity,  gives us much greater ability to quickly find all of your health information and track your visits and treatments more accurately than can be done with paper. 

You can also create your own Personal Health Record through our website.  This exciting new program is available free of charge to our patients.  You can generate your own health record and store it on our secure HIPAA compliant practice portal.  You can update your information at any time and even upload medical records from other doctor''''s offices. You can input your new medications and any changes in your  health.  You can even monitor your important vital signs such as blood pressure and weight and chart  your progress.   Importantly, you can custom create a Continuity of Care record which you  can print out and place in your wallet for ready reference.  Importantly, your Personal Health Record travels with you when you travel or visit other doctor''''s offices.  All you need is an internet connection and you will be able to download all the information you have placed on the site.  In this way, your medical records are stored securely for your use at home or on the road!
